Yuberky is a 45-year-old woman, who resides in the Baltimore City area of Maryland with her family. At a young age, Yuberky was diagnosed with a severe intellectual disability and developmental disorder of speech and language. As a result, Yuberky has depended on support from her family, friends, and care team to meet her daily needs.
Since meeting her Support Planner, Brandi, she has been very happy working with her. “Yuberky is always smiling and a joy to be around,” Brandi said. Brandi has played an integral role in helping Yuberky obtain and maintain services through the Community First Choice program. Those services include Personal Assistance, Nurse Monitoring, Environmental Adaptations, and Adult Medical Day Care. In addition to these services, Brandi has also assisted in other supports to include Physical Therapy, Transportation, Speech Therapy, and application assistance (MD Energy Assistance Application).
Most recently, Brandi successfully helped Yuberky find a new Adult Medical Day Care (AMDC) program better suited to her needs. Yuberky’s previous AMDC was far from home, resulting in long hours spent on the transport bus each day. This issue came to light during one of Brandi’s home visits, when Yuberky’s mother, Lydia, and her brother, Joan expressed concerns about the lengthy rides and the lack of adequate climate control on the transport vehicle, which made the trips even more uncomfortable for Yuberky.
It became Brandi’s goal to find a closer and more suitable AMDC for Yuberky to attend. After some time spent searching, she found the perfect location for Yuberky. The facility she now attends is much closer to her home. As a result, Yuberky spends minimal time on the transportation vehicle.
Since moving to this new AMDC facility, Yuberky’s brother, Joan, has had nothing but wonderful things to say about them. In addition to the location being close by, they participate in a multitude of activities with their clients to include field trips, dances, sporting events, crafts, and much more.
John expressed to Brandi, ‘Thank you so much for helping my sister; she is much happier with this new location.’
With ongoing support from her family, friends, Support Planner, and other agencies, Yuberky continues to enjoy a safe and happy lifestyle, which is reflected in her smile, which seems to grow brighter with each new day.
