Meet Michael! After facing life-changing illnesses at the young age of 17, he lost many of his teeth, his ability to speak, and his independence.
Through Service Coordination, Michael met Rebecca, a dedicated Supports Planner. Together with Ryan, a Coordinator of Community Service, Rebecca helped Michael:
✅ Enroll in the Community First Choice program for daily living support
✅ Access transportation resources for greater independence
✅ Secure funding for dentures and a shower chair to improve safety
✅ Achieve his dream of taking ASL classes at his local college
Even after a pneumonia setback, Rebecca ensured Michael received meals, nurse monitoring, and personal assistance to recover. Today, Michael lives independently in his apartment, with Rebecca continuing to check in monthly.
