Yes, Community-Medical Assistance is the requirement for eligibility purposes.
WHAT DOES THIS SERVICE PRIMARILY HELP WITH?
Personal Attendant Care is one of the core services in the Supports Planning Program. An example if someone needs assistance with their Activities of Daily Living (ADL’s) such as bathing, dressing and assistance around the home, a person might be eligible for this program.
I HAVE A SERVICE COORDINATOR? CAN THEY WORK TOGETHER WITH A SUPPORTS PLANNER?
Yes! We encourage collaboration between the two, the only caveat is that DDA services and Support Planning Services cannot overlap. Your Supports Planner can help you with this process.
IS THERE AN AGE LIMIT?
We currently are serving a 7-month year old and a 101-year-old.
IS THERE A WAITLIST FOR SERVICES?
No, there is no wait list for services
CAN SUPPORT PLANNERS HELP WITH ACCESSIBILITY ISSUES (STAIR GLIDE, GRAB BARS, RAMPS) IN THE HOME?
Yes, they can if they meet a certain level of need as determined by the nursing assessment.
WHY WOULD SOMEONE PICK SERVICE COORDINATION IF YOU JUST HAVE OFFICES IN OWINGS MILLS AND FREDERICK?
Supports Planners are a mobile workforce and we have Supports Planners who live in or close to you in your community.
I’M THE PARENT OF A PARTICIPANT AND I WANT TO BE THE PAID PROVIDER.
You can be the provider if you are not the authorized representative on the Plan of Service.
I’M THE GUARDIAN CAN I PROVIDE SERVICES TO THE PARTICIPANT?
No, I’m sorry you cannot.
CAN MY SERVICE COORDINATOR BE MY SUPPORTS PLANNER?
No, the two programs exist in two separate waivers and they are two separate programs of service. Question: That sounds confusing. Answer: We will do our best to explain this to you in person at the Plan of Service Meeting!
HOW IS SUPPORTS PLANNING DIFFERENT FROM SERVICE COORDINATION?
They really are not that different, both programs provide Case Management in a Person-Centered Model and both programs seek to help guide people into services. The key difference is that Supports Planning is more medically based and Service Coordination is more community based.
WHAT IS PERSON-CENTERED PLANNING?
Person Centered Planning enables people to choose their own pathways to success; the planners simply help them to figure out where they want to go and how best to get there.
